Payments for Verifications, Rescores, and Replacement Certificates

The ABP accepts VISA, Mastercard, and American Express for Verifications, Rescores, and Replacement Certificates.

The following ABP forms must be downloaded, completed using Microsoft Word, and mailed with the payment or Credit Card Authorization form.

• Verification of ABP Certification Form
  
• Form to Request Rescoring of Examinations
  
• Form to Request a Replacement Certificate
  
• Non-US Credit Card Authorization Form
  

Payments for primary and subspecialty examinations (applications and registrations) and MOC must be made through the ABP on-line system.
