To request a replacement copy of a certificate, click the "Replacement Certificates" link in the Payments menu.
Selecting a Certificate
The list of certifications is divided into Primary and Subspecialty. To select between the two certifications, click the Primary or Subspecialty radio button at the top of the page.
Next, in the list of certifications, click the check box next to the certificate that you are requesting a copy of. You can select multiple certificates. Each copy will be charged the standard replacement fee individually.
Checking Out
When you have finished selecting the certificates, click the "Checkout" button, located at the bottom of the page. The Checkout page is displayed, where you will enter your mailing and billing information.
